What Does a Home Care Package Assessment Involve?
If you’re interested in organising care via a Home Care Package, you’ll need to undergo an assessment with My Aged Care.
So, what exactly does a Home Care Package assessment involve? Let’s find out.
Applying for a Home Care Package (HCP)
Applying for a Home Care Package is relatively stress-free. However, there can be a bit of a wait between when you start your application, and when you begin receiving your services. So, keep the wait time in mind, and apply for your HCP as soon as possible.
To start organising an HCP, you’ll need to contact My Aged Care either via phone or online. My Aged Care will ask you a few simple questions about your needs and then organise an assessment if they think you’ll qualify.
What Does a Home Care Package Assessment Involve?
Each older Australian has unique needs, so HCP funding shouldn’t be one-size-fits-all. This is why an assessment is necessary—it helps My Aged Care properly understand you and your needs.
Your assessment will be carried out by the Aged Care Assessment Team (ACAT). A friendly member of the ACAT team will visit you in your home (or in the hospital if you’ve been admitted) and ask questions about your day-to-day life. This could include the areas of daily living you struggle with, whether you have family members and friends to support you, and whether you’re experiencing illness or injury. Depending on your situation, the ACAT member may also ask to speak with your GP to better understand your needs and medical history. You are more than welcome to have a loved one present during your assessment.
Generally, an ACAT assessment will last between 45 minutes to an hour and a half. Then, after your assessment is complete, it could take around two to six weeks to receive your results.
I’ve Been Approved for a Home Care Package, What’s Next?
After your assessment, you’ll receive a decision letter from My Aged Care detailing whether you’re eligible for an HCP, and which home care services you’re eligible for. Now it’s time to find a home care provider! Every older Australian has the right to choose their own home care provider. This means you have complete control over selecting a provider that best suits your needs and preferences. So, find a local provider who offers care via a Home Care Package (and offers the services you’re eligible for), and get in touch. Together with your chosen provider, you’ll create a tailored care plan to help you live comfortably at home.
Note: If your needs change and you could benefit from a higher level of funding, you can apply for a re-assessment. The ACAT member will revisit you, discuss these changes, and make any required adjustments.
