Report an Incident to Simply Helping

Simply Helping prioritises the safety and wellbeing of all service users and will notify the Aged Care Quality and Safety Commission or the NDIS Quality and Safeguards Commission of reportable incidents.

If you’re a service user, family member or friend of a service user, or a member of our support staff and want to report an incident, complete the online form by clicking the button above.

Please note, for incidents that are deemed as reportable (see below) contact your service provider immediately.

What are reportable incidents?

  • Death or serious injury
  • Unreasonable use of force
  • Unlawful sexual or physical contact, assault, or sexual misconduct
  • Abuse, including psychological or emotional abuse
  • Neglect
  • Stealing or financial coercion by a staff member
  • Inappropriate use of restrictive practices
  • Unexplained absence from care/missing care recipient

Additional Information

If you are an older Australian receiving Commonwealth-subsidised aged care and services:

The Serious Incident Response Scheme (SIRS) is an initiative to help prevent and reduce the risk and occurrence of incidents of abuse and neglect of older Australians receiving Commonwealth-subsidised aged care and services.

The scheme requires aged care providers to notify the Aged Care Quality and Safety Commission about reportable incidents. Aged care service providers must also manage and take reasonable action to prevent incidents with a focus on consumers’ safety, health, wellbeing and quality of life.

If you are concerned about the care you or your loved one is receiving, you can raise a concern or make a complaint with the Aged Care Quality and Safety Commission on 1800 951 822.

If you are an NDIS Participant:

Under the NDIS Commission, all registered NDIS providers must have an incident management system to record and manage incidents. NDIS providers must notify the NDIS Commission of reportable incidents, including the death, serious injury, abuse or neglect of a person with disability and the unauthorised use of restrictive practices.

If you have a concern about your current NDIS supports or services, you can make a complaint to the NDIS Quality and Safeguards Commission about your NDIS Provider. Your family or friends can also talk to the NDIS on your behalf. You can call the NDIS Quality and Safeguards Commission on 1800 035 544.
